PatchSFO is used to patch the EBOOT files to make them compatible with firmware 2.80. It scans all applications in ms0:/PSP/GAME/ and patches them so that they will start with xLoader.

The program is used to make it so that the PSP's standard menu (XMB) will recognize homebrew as valid, otherwise you get the 'corrupted data' icon. The XMB will still fail to start the homebrew, but it makes it think that it's a bit more valid than it really is. This then means that the XMB will let you try to start the homebrew with xLoader. So basically it makes sure that anything that runs with eLoader can also be attempted with xLoader.

It can't change homebrew that wouldn't normally be able to run on your current firmware version.

It can also patch some of the official downloadable demos so that they run on 2.80.
